The goal of the CHN is to provide a springboard for community building, collaboration, and communication between Capital Region consumers and their providers and public health leaders.

Why does the CHN need me?
Every day there are new programs and decisions being made to make communities healthier. These decisions are always made with you in mind, however, sometimes they aren’t as successful as planned. Tell us what you need to make you and your community healthier.
Why should I join the CHN?
The CHN works hard to make your voice as loud as possible when it comes to health decision making and planning. In exchange for your valuable opinions, we are happy to offer free in-person help to anyone you know seeking help with getting care, self-managing their health, or obtaining social service benefits. We also offer an opportunity for you to become more involved in health decision making and want to help you accomplish your health goals. What happens after I join the CHN?
We will email you 3-5 (optional) short surveys a year. After you’ve submitted the survey, you will be entered into a drawing to win one of multiple gift cards to various locations. The CHN will also share survey results with you along with any related health resources and news. Your email address will not be shared.
I have a question about the CHN or I have a question about what my county is doing to improve my health, who can I contact?
You can send any questions you have about the CHN or the public health services available to you to CHN@hcdiny.org. You can also call (518) 486-8401 – ask for Jordyn.
I want to be more involved, how can I learn more?
You can become more involved in a couple of ways.
- Email us at CHN@hcdiny.org or call (518) 486-8401 and ask how you can become even more engaged
- Subscribe to the HCDI newsflash and PHIP newsletter
- Look out for informational emails from us describing opportunities to bring the consumer voice to local public health meetings.
- Follow our social channels (I can list small icons with links) for local health events, health self-management tips, community support opportunities
I want to take the surveys but I do not have regular access to an email address, computer, and/or the internet.
There are many ways to take the surveys at your convenience.
- Go to our CHN page (web link) and click on the ’take the survey’ button. No email needed.
- No computer access? Call us at (518) 486-8401 and ask to take the survey by phone.
How do you choose survey topics and what do you do with the survey results?
Our surveys are made to answer questions that our partners (hospitals, providers, health care leaders) have about you and what you think.
The results are used by those health partners to improve health delivery in the capital region.
