Primary Care Provider Survey
Our goal here was twofold. We asked participants to assess a Primary Care Provider (PCP) brochure created to motivate clients to find a regular PCP. We also asked survey-takers to tell us why they value their PCP relationship.
Community Health Needs Assessment (CHNA) Survey
Our first 2019 survey is a call to our stakeholders and the public to review the ease of navigation and readability of the CHNA —data analysis of the health needs in the Capital Region.
Care Selection Tools Survey Results
Our 2018 Quarter 2 survey measures the user-friendliness of our Healthcare Team and Care Selection Tools in guiding consumers to get the right health care when they need it.

Social Determinants of Health Survey Results
Our quarterly topic was Care Coordination with a focus on social determinants of health. A total of 60 individuals completed the survey.
Obesity and Physical Activity Survey Results
Our quarterly topic was Obesity with a focus on physical activity. A total of 110 individuals completed the survey.
Diabetes Survey Results
To complement our diabetes equity report, we conducted a survey on Type 1, Type 2, and Prediabetes . A total of 47 individuals completed the survey.

Heart Disease & Stroke Equity Report
This report on Cardiovascular Disease presents national, state, county, and neighborhood (Zip code aggregate) level information by health equity for heart disease and stroke. State and Regional data on high blood pressure are also available.
Food and Housing Equity Report
This report contains maps, tables, and highlights by county and census track for the social determinants of food and housing.
Diabetes Equity Report
This report highlights diabetes in the Capital Region by age, income, race, and geographic areas.
Obesity and Physical Activity Equity Report
This report looks at indicators relating to obesity, nutrition, and physical activity by New York State, the Capital Region, and school district.
