The 2016 Community Health Needs Assessment contains county and zip code group level analysis of causes of death, hospitalization and emergency room use for Albany, Columbia, Greene, Rensselaer, Saratoga, and Schenectady counties. Ten-year trends, gender, ethnicity and age patterns were also examined when available.
Regional (10 County) DSRIP Needs Assessment
Analyses on regional and local health needs, availability of providers and community-based organizations, and consumer input via survey, focus groups, and community listening sessions, with more in-depth analyses of the Medicaid and uninsured population needs. Included counties: Fulton, Montgomery, Saratoga, Schenectady, Albany, Rensselaer, Columbia, Greene, Dutchess, and Ulster.

Community Health Conversations Report
In November 2022, Healthy Capital District convened Community Health Conversations to clarify obstacles to health for populations that experience disparities and prepare for the movement toward value-based payment structures that incorporate medical and social care. Over the course of 7 months, 589 individuals from 159 different organizations registered to attend Community Health Conversations. Throughout our Community Health Conversations, there are clear regional priorities that continue to present as key barriers to health and wellness, as presented in this report.
Indicator Matrix
The Public Health Indicator Matrix presents a list of nearly 200 commonly used public health indicators for New York State, New York State excluding NYC, the Capital Region, and each of the six Capital Region counties. The indicators are listed by the Prevention Agenda Priority Areas. The Public Health Indicator Matrix contains both rates (age-adjusted or age-specific) as well as the population at risk and identifies if the counties fall into the 3rd or 4th risk quartile for all NYS counties.
