HCDI has assembled a comprehensive regional health profile with ten (10) years of trended health risk factor data displayed at the zip code level. The profile is used by our sponsors and others in targeting community health programs and in leveraging public funds.

Collaboration among partners in the collection and reporting of health statistics has expanded the Health Profile.  The Profile helps sponsors in the preparation of County Community Health Assessments and Provider Community Service Plans.
