Health Insurance Application Assistance

The Facilitated Enrollment Program provides free help to individuals and families who wish to apply for Child Health Plus, Family Health Plus, Medicaid and PCAP at convenient community locations.

Our helpful and friendly staff will guide you through the application process for Child Health Plus and Family Health Plus and answer any questions that you may have. Meeting with a Facilitated Enroller can help you avoid long waiting times at your local Department of Social Services.
